New, non-severe headaches or digestive concerns that are not accompanied by emergency warning signs.

Explain the general reason for your request so the team can review scheduling availability and provide appropriate direction.

Complete an Evaluation

Your physician reviews your symptoms, relevant history and clinical findings.

Determine the Care Plan

Recommendations may include treatment, testing, monitoring or a more appropriate level of care.

Follow Up if Needed

Additional review may be recommended if symptoms persist, worsen or require continued monitoring.

Clear information helps the office understand your request and identify whether an available primary-care appointment may be appropriate.

Note when you first noticed the concern and whether it appeared suddenly or gradually.

How the symptoms have changed

Be prepared to explain whether the concern is improving, remaining stable or becoming worse.

What you have already tried
